1. All Teams INCLUDING OUT OF STATE Teams must fill out an ILLINOIS MEDICAL WAIVER!
2. All Teams must also fill out an IYSA Tournament Roster.
3. Illinois Guest Player Forms can only be used by Illinois Teams.
Out of State Teams Must use their State Associations Guest Player Procedures.
