All Teams are Required to Mail (2) Seperate Copies

1. ( 2) ILLINOIS MEDICAL RELEASE FORMS:
REQUIRED FOR ALL TEAMS! (INCLUDING US CLUBS TEAMS & OUT OF STATE TEAMS). No Exceptions for any teams, required by the IYSA!
2. (2) Stamped/Approved Copies off your State Roster/League Roster.
3. (2) REQUIRED FOR ALL TEAMS- Fully Filled Out PUMA Champions Cup Tournament Rosters.
4. (2) Travel Permits for any out State Team. Along with (2)Travel Permits for any Out of State Guest Players your team may have.
5. (2) Your States Guest Player Form for any Teams using Guest Players.
